I agree with what Comrade K said. If "never die" means immortality, then I'd have to consider halving my lifespan. But if it only means no ageing, then I'd definitely choose an age, because eventually I'd be killed by an accident/speeding bus. And with an indefinite lifespan, you'd have plenty of time to seek inner peace, and find true happiness. That, and with over 500 years experience, you'd do really well in gameshows.
Now, what age to choose... Around twenty, I think. The time when your body's fully developed, but hasn't started ageing yet. Then you'd be in peak physical condition forever.

For me, this is an interesting question, because if I were to choose an age, it would be between two different options... Part of me longs for the carefree nature of being a child, to which I can think of no better age than 2. May seem young, but I have this strongly positive feeling about that age. The other part of me longs to be self-sufficient. For this, I would likely choose 22-25. It'd be a tough decision between those two. Of coarse, if I didn't have the 'never die' explained more thoroughly to me, I'd take the halved life expectancy. I would need to know exact details about that, whether things like getting hit by a bus would kill me, or if I was immortal entirely.
If you've done things right, people won't be sure if you've done anything at all.

As to why someone might refuse immortality, if you've seen the repulsive Death Becomes Her, you have an idea. Of course, that's just one conception of it. "Something Happens" has a better explanation (which I don't have time to find) in a cartoon ad for a medicine that grants immortality, listing numerous logical, unpleasant side effects near the end. One of them was the agony one would experience when the Sun burns out.
