What did I do wrong?

Vest appears to be bulging outward as if Ozy had a MANLY CHEST that was pushing it away. The head is a bit deformed and looks like the old fashioned ozy and the new ozy combined, the nose is too big, and the feet seem rounded.

But you're not terrible so don't kill yourself or anything.

Bigger canvas will mean more distortion in the end, and it actually is vector, or quite possibly vexel.
