I get the impression that DCS didn't think of the punchline until he had a rough idea how the first three panels would look.
This is not a criticism. While "waiting until the last second" can lead to cheap jokes sometimes (not this time), I think they're better on the whole than what I see in many comics, where the cartoonist goes to too much trouble to set up for the punchline.
